Registration Information

You will need:

• Shot Records
• Immunization Form/DSPA Form 122.1: This form may be obtained from any of the clinics or the US Naval Hospital on Camp Lester. You must take your child's shot records and/or medical records to the medical facility in order for this form to be completed. You do not need to make an appointment.
• Passport or Birth Certificate for Kindergarten
• PCS Orders
  Air force -----Copy of orders with the child's name on it.
  Marines-------A copy of Area Clearance for each child and PCS Orders.
  Navy----------Pg. 2, Dependent Entry Approval, PCS Orders, and Projected Rotation Date(PRD)
  Army----------Copy of PCS Orders with child's name on it
  Civilian--------Verification of Employment and latest SF-50
• Address of previous school in case the school needs to request records.
• Proof of child's social security number
• Proof of Residency: Housing assignment letter or copy of lease and map for off base housing
• Registration Form 600: The form can be found at http://www.pac.dodea.edu/publications/downloadable/DoDEA_Form_600.doc
  
  
  Kindergarten children must be five years old on/or before 31 October to be enrolled in Kindergarten.
  
  First grade children must be six years old on/or before 31 October to be enrolled in first grade.
